Summary
The Community Resource Team (CRT) is a joint service provided by the Health Board and Neath Port Talbot Council Social Services.
The team helps adults who are living in the Neath Port Talbot area with the support they need to retain their independence in their own homes.
The support can include acute clinical care, home alterations and equipment, physiotherapy and speech and language therapy, help with diet, help with washing and dressing and help with medicines.
The team can support people to come out of hospital and back to their own homes. They also help to prevent hospital and care home admissions by assessing the level of need and providing the right assistance.
How to access the service:
1. Referral by GP / Health Professional
You can be referred to the service by your GP, hospital, district nurse or social worker.
2. Self Referral via Telephone / Email
